Churchill Home Care Hospital To Home Care

Churchill Home Care can help assist you with your return home from the hospital. Our hospital to home care services vary and will depend on the level of care tath you require, such as Hourly Care, Night Care and Live-In Care. Our Hourly Care starts from just £18 per hour and our Live-In care starts from £725 per week.

Our hospital to home care service can be tailored to suit your needs and is available on short notice, as well as on a respite care basis to assist you on a short term basis while you recover.

We will always work with you to develop a bespoke care plan to ensure that you receive the right level of support and assistance on your return home from the hospital. Usually, hospitals will not discharge an elderly patient unless they can return home safely with the right care, support and assistance in place. For people with dementia, returning home as soon as possible is incredibly important, as an unfamiliar environment can be upsetting and cause additional confusion, disorientation and distress.

Churchill Home Care provides hospital to home care for patients currently in hospital and is looking to return home, patients who are due to go into hospital and will require assistance and support upon returning home and fro patients who have already returned home but require extra assistance and support.

Different hospitals have different discharge procedures, but typically if you are going into to hospital for a planned procedure then your discharge procedure is decided upon before you go into to hospital, so you know what to expect and what support and any aids you may need can be arranged. If you go into hospital on an emergency basis that is unplanned then the discharge planning begins from when you first go in.

Churchill Home Care will be able to work with you to ensure that the care plan that is put in place offers all the support and assistance that you need in order to be discharged safely. We will match you with a carer that is suitable for the level of care that you require and is experienced with any aids or equipment that may need using.
